Can't connect with the company to reply to the verification FAX???

Please help us help you.

(1)You need to file a complaint with the Federal Trade Commission via the internet. Under "subject of complaint", choose "Health". At the bottom of the form there is an area to summarize your problem.

Be very brief in summarizing your problem [i.e. can't communicate with company, or maybe the FAX # is always busy, or maybe the FAX # is no longer in working order, or need to talk with someone regarding the problem.

(2)Send an email to the AOA office to Mr. Jon Hymes - Washington Office Director at Email: jfhymes@aoa.org so that AOA can track the number and types of problems. Please list the day you filed your complaint with the FTC and the summary of your problem.

(3) Send a cc: copy to the FOA office to Mr. Ken Franklin Ken@floridaeyes.org so we can develop a complaint file also.
