Notification of a change in Practice Location



Question: I just recently moved my optometric office. Do I have a duty to notify the Florida Department of Health whenever I change my mailing address or place of practice?

Answer: Yes. Rule 64B13-3.019, Florida Administrative Code, adopted by the Florida Board of Optometry expressly provides that it is the optometrist’s duty to provide the Department of Health with written notification by certified mail of the optometrist’s current mailing address and place of practice no later than ten (10) days after a change thereof. “Place of practice” means the address(es) of the physical location(s) where the licensee practices. An optometrist’s failure to comply with this requirement is a basis for disciplinary action against the optometrist’s license.
